Ijasah S1<\/li><li>Scan Transkrip Nilai S1<\/li><li>Scan Kartu Identitas KTP\/SIM<\/li><li>Scan NPWP (bagi yang memiliki)<\/li><li>Scan SK Kepegawaian Terakhir (SK Kepegawaian terakhir bagi guru honorer adalah SK dari Bupati\/Provinsi bisa tambah dengan SK pembagian tugas sekolah terakhir.
